APPLYING FOR AN ADULT PASSPORT ON A FULL (PC8) PASSPORT APPLICATION
You must use a full PC8 passport application if you are applying for your first adult passport, you have changed your name, your passport expired more than two years ago or your passport has been lost, stolen or seriously damaged.
• A completed PC8 Overseas Application for an Australian Passport (please use BLACK Ink and BLOCK LETTERS) with the following details:
o Date of travel (if you are travelling in the next two months)
o Personal details – your current Family and Given names, Sex, Date of Birth, Place of Birth and Country of Birth
o Details of the documents you will present to confirm your citizenship
o Change of Name documentation if your name has changed
o Your mother’s family name/surname (only) at the time of her birth
o What type of passport you are applying for
o Your contact details
o Details for someone that can be contacted in case of an emergency
o Your residential address
o Details of your current Australian passport (if applicable) and if this or another Australian passport was lost or stolen in the past five years
o A guarantor must fill in the section and sign
o Details of your Australian Driver’s Licence or Australian Birth Card if you are presenting either one as proof of your identity
o You must read then sign and date the declaration and give your full name
o Two passport photographs that meet Australian passport requirements. One of which has been signed by the guarantor
o You must also sign within the box ‘Signature to go in passport’
• Your Original Birth Certificate (if applicable)
• Your Proof of Citizenship
• Passport application lodgement fee
• Your most recently issued Australian passport (if applicable)
• A combination of Proof of Identity Documentation
If you are renewing a passport that expired more than two years ago, you can lodge your application by mail. Please send it to:

You will need to lodge the application in person if you are applying for your first passport as an adult (after you turn 18), you have lost, stolen or seriously damaged your passport or you are changing your name.
