APPLYING FOR A PASSPORT FOR A MINOR

In order to lodge an application for a passport for a Minor (a child under 18 years of age) you will need to provide the following documentation:

• A completed PC8 Overseas Application for an Australian Passport (please use BLACK Ink and BLOCK LETTERS) with the following details completed:

o Date of travel (if you are travelling in the next two months)
o Personal details – your current Family and Given names, Sex, Date of Birth, Place of Birth and Country of Birth
o Details of your Australian Birth or Citizenship Certificate to confirm your Australian Citizenship 
o Change of Name documentation if your name has changed
o Your mother’s family name/surname (only) at the time of her birth
o What type of passport you are applying for
o Your contact details
o Details for someone that can be contacted in case of an emergency
o Your residential address
o Details of your current Australian passport (if applicable) and if this or another Australian passport was lost or stolen in the past five years
o A guarantor must fill in the section and sign
o Witnessed parental consent from those who have parental responsibility
o The parent lodging the application on a minor’s behalf must sign and date the declaration and give their full name
o Two passport photographs that meet Australian passport requirements. One of which has been signed by the guarantor 
o If over 10 years of age, the applicant is expected to sign within the box ‘Signature to go in passport’

• The child’s Original Birth Certificate 
• The child’s Proof of Citizenship
• Passport application lodgement fee
• The child’s most recently issued Australian passport (if applicable)
• One form of official identification (foreign passport, verblijfsvergunning (residents permit), driver’s licence etc.) of the parent lodging the application on the child’s behalf with their name and signature 

If you are applying for your child's first passport, you will need to lodge the application in person at the Australian Embassy in The Hague. 

The Consular and Passports Section is open from 09.00-13.00, Monday to Friday

If you are renewing your child's passport, you may be eligible to lodge the application by mail.  Please ensure that you include original documents with the application.  Please send the application to:
