Certificate of Good Conduct

The Australian Embassy in The Hague can assist with your application for a Dutch Good Conduct Certificate.

The relevant form can be found on the website of the Dutch Ministry for Justice, click on the link to download and print the Certificate of Good Conduct application form ('Aanvraagformulier Verklaring omtrent het Gedrag'). Complete your personal details, sign the form and send it together with a self-addressed, stamped envelope to the following address:

The Australian Embassy will complete the section for the requesting organisation and will return the form to you.  The application should then be lodged at your local town hall (gemeente). They will issue the Certificate of Good Conduct.

If you are not in the Netherlands to lodge your application personally, you will need to authorise someone to lodge it on your behalf. Procedures on how to do so can be found on the website of the Dutch Ministry for Justice.
